For Pharmacists and Pharmacist Interns

For EMTs, Paramedics, and Radiologic Technologists
You may request Licensure Verification online or in writing. There is no fee associated with Licensure Verification for your profession.

For All Other Health Care Professions
You may request licensure verification online or in writing. There is a $25.00 processing fee.

If you are applying for a Florida license, you do not need to submit a verification request for your Florida license.
