How to order a Clay County death certificate from the Clay County Health Department

Certified Copies of Death Certificates for deaths occurring in Clay County from 1997 to present are available. Anyone may obtain a certified copy of a death certificate, but only the immediate family may obtain a certified copy with the cause of death. Valid photo I.D. is required when requesting the cause of death. The fee is $10.00 per copy. You may download an application from this web site, and apply in person or by mail.
