Lafayette County Community Health Improvement Plan

In 2011, the Lafayette County Health Department received grant funds from the Florida Department of Health to work on a community health improvement project. The health department partnered with the Suwannee River Area Health Education Council to complete a community health assessment and improvement plan. This collaborative effort involved representatives from the health professionals, community organizations, businesses, and concerned citizens. A comprehensive, county-wide Community Health Assessment using a nationally recognized approach, called MAPP (Mobilizing for Action through Planning and Partnerships) was completed over a six-month timeframe. The data from the collaborative community effort resulted in the 2011 Community Health Improvement Plan (CHIP). This CHIP serves as a framework for health improvement in Lafayette County and includes strategic health issues and actions and goals to achieve improved health. The Community Health Improvement team welcomes input from our community. Lafayette County Needs Assessment

The Lafayette County Needs Assessment Report consists of demographic, socioeconomic and health status information that will be used to identify areas where targeted interventions and policy changes may have the greatest impact.


Lafayette County Annual Report 2009

The Lafayette County Annual Report for 2009 presents a summary of the health department's impact on the community through the medical services provided, the Healthy Communities Healthy People initiatives, disease intervention activities, environmental health and prevention programs, and H1N1 and public health preparedness efforts.
