School Health Vision Services Region List

1.  Florida Heiken Children's Vision Program (FHCVP) - The Florida Department of Health has entered into contract (#COH9L) with FHCVP for school year 2011/12.  FHCVP is contracted to provide vision services to eligible students enrolled in and attending public schools in counties within its home base, and Regions Two and Four (refer to the School Health Vision Services Region List).  Eligible students in these counties can be referred to FHCVP for services.

Florida's Vision Quest (FVQ) - The Florida Department of Health has entered into contract (#COH9M) with FVQ for school year 2011/12.  FVQ is contracted to provide vision services to eligibile students enrolled in and attending public schools in counties within its home base, and Regions One and Three (refer to the School Health Vision Services Region List).  Eligible students in these counties can be referred to FVQ for services.
